◾️Chapter 1: General Provisions
Article 1 (Purpose)
These Terms and Conditions set forth the terms for the use and membership registration of the "WIT Health and Sports Science Center" (hereinafter referred to as “the Facility”), operated by Imagawa Shoji Co., Ltd. The purpose of these Terms is to ensure that all users can enjoy the Facility safely and comfortably.
We sincerely request your understanding and cooperation in maintaining a clean and welcoming environment, as we aim to serve as a place that connects medicine and the future of health.
Article 2 (Scope of Application)
These Terms apply to all users of the Facility. Individuals who register as members are deemed to have agreed to these Terms.
◾️Chapter 2: Membership System
Article 3 (Definition and Types of Members)
- A "Member" refers to an individual or organization that agrees with the Facility’s objectives and accepts these Terms.
- A "Monthly Member" refers to a Member who uses the Facility under a monthly membership plan.
Article 4 (Eligibility for Membership)
- Conditions for Enrollment
Individuals who meet the following conditions may become Members:
- Agree with the objectives of the Facility and accept these Terms.
- Are not prohibited from exercising by a physician (unless special permission is granted).
- Minors (high school students or younger) must obtain the consent of a parent or guardian.
- Restrictions on Membership
Individuals who fall under any of the following categories are not eligible for membership:
- Members of organized crime groups or other antisocial forces.
- Individuals whose behavior, speech, or appearance (including visible tattoos) are deemed inappropriate by societal standards.
Article 5 (Membership Registration and Change Procedure)
- Membership registration is conducted via the “AZZIST” system (operated by AZCARE Inc.): https://wit.azzist.jp/transaction
- Members must promptly update their personal information via the member website.
◾️Chapter 3: Facility Use Rules
Article 6 (Eligible Users)
Only individuals who are not prohibited from exercising by a physician may use the Facility.
Article 7 (Operating Hours and Closed Days)
- Operating hours are posted on the Facility’s official website.
- The Facility will be closed on the following days:
- New Year's holidays
- Days designated for special maintenance or repairs
- Days when operation becomes difficult due to natural disasters or other force majeure events
Article 8 (Rules for Facility Use)
When using any area of the Facility (changing rooms, lockers, training areas, conditioning areas, or private rooms), users must observe the following rules:
- General Rules
- Be considerate of other users and avoid causing disturbance.
- Handle equipment and furnishings with care to prevent damage.
- Dispose of trash in designated bins and help maintain cleanliness.
- Notify staff immediately if you feel unwell.
- Do not operate lighting, air conditioning, or other equipment without permission.
- Changing Rooms and Lockers
- Store clothing, footwear, and personal belongings in designated lockers.
- Be careful not to leave belongings behind and keep vanity areas clean.
- Training Area
- Use training machines safely after reading the usage instructions.
- Conditioning Area
- Use equipment and supplies only with staff permission.
- Private Rooms
- Use in a manner that does not disturb other users.
◾️Chapter 4: Fees and Payments
Article 9 (Membership and Service Fees)
- Members must pay the applicable membership and service fees for use of the Facility.
- Payment is made in principle by credit card.
- All payments are processed through the “AZZIST” system (operated by AZCARE Inc.).
- If you are unable to use a credit card due to special circumstances, please consult with Facility staff.
- Monthly membership fees are billed on the 23rd of the preceding month.
- If the credit card payment fails due to exceeding the credit limit, reattempts will be made on the 27th, 1st, 4th, 7th, and 10th.
- Monthly Members are required to pay the monthly fee regardless of actual use, until membership is cancelled.
- If fees remain unpaid for two consecutive months, the Member will lose membership status and be considered withdrawn. Any unpaid fee will still be charged.
- Fee changes will be notified via the official website, written notices by mail, or in-facility postings at least one month in advance.
- Membership and service fees are non-refundable.
- Changes and cancellations of reservations are subject to our separately established cancellation policy.
Article 10 (Membership Card)
- A membership card will be issued to Monthly Members as proof of membership.
- Please present the membership card or your member site’s My Page when using the Facility.
- Membership cards are non-transferable and may only be used by the registered Member.
Article 11 (Group Session Tickets)
- Group sessions may be purchased using session tickets.
- Resale or transfer of purchased tickets is strictly prohibited.
◾️Chapter 5: Leave of Absence and Withdrawal
Article 12 (Leave of Absence, Reinstatement, Plan Change, and Withdrawal)
- Leave of Absence
- Monthly Members wishing to take a break must apply via the member site by the 10th of the preceding month.
- During the leave period, Facility access is not permitted, and a leave fee applies.
- Reinstatement
- To reinstate membership, apply via the member site by the 10th of the preceding month.
- Plan Change
- Requests to change membership plans must also be submitted by the 10th of the preceding month.
- Withdrawal
- Monthly Members who wish to cancel their membership must do so via the member site by the 10th of the preceding month.
◾️Chapter 6: Collaboration with University of Tsukuba Hospital
Article 13 (Information Sharing)
- The Facility may share personal information with the University of Tsukuba Hospital for the purpose of health management of its users.
Article 14 (Handling of Personal Information)
- Personal information is managed appropriately.
- Users have the right to request access to or correction of their personal information.
◾️Chapter 7: Disclaimers
Article 15 (Liability, Theft, Lost and Found)
- Liability Related to Facility Use
- The Facility assumes no liability for any accidents involving Members or third parties.
- Theft
- Users are responsible for their own belongings. The Facility is not liable for theft.
- Lost and Found
- Lost items will be managed in accordance with the “Kirinoha Mall Lost Property Handling Protocol.”
◾️Chapter 8: Amendments and Notices
Article 16 (Amendment of Terms)
These Terms may be revised as necessary. Any changes will be announced through the official website, in-facility postings, or written notices by mail.
Article 17 (Jurisdiction)
All disputes related to these Terms shall fall under the exclusive jurisdiction of the Tsuchiura Branch of the Mito District Court as the court of first instance.
